Presence of symptomatic cardiac insufficiency

Presence of severe gastrointestinal involvement (bowel perforation, bleeding, and pancreatitis)

Presence of renal insufficiency (creatinine > 150 mmol/L)

Absence of ear, nose, and throat symptoms
